+ \`\_Xtoc` `{<level>}{<type>}{<number>}{<title>}` (in `.ref` file) reads the

+ the record to the table of contents. It opens group, reduces `\_leftskip`,
+ `\_rightskip`, runs the \^`\everytocline` (user can customise the design of TOC
