[CTWILL] Add macros 'pdfctwimac' for PDF creation.
* Internal links along section numbers work perfectly.
* Bookmarks/outlines for starred sections work perfectly.
* Additional bookmark for 'Names of the sections' is flaky:
- pdfTeX points 'NOS' to 'section 1' (as intended);
- XeTeX doesn't link the main bookmark anywhere (as planned!),
but it adds an extra spread of empty pages with a mini-index for
the very last section.
Note that pdfTeX's 'ctwill.pdf' causes serious problems with 'MuPDF'
on Android 7.1. XeTeX's 'ctwill.pdf' works fine on the smartphone.
Note also that the CTWILL macros (irrelevant if the original
'ctwimac.tex' or the new 'pdfctwimac.tex') tend to produce extreeemely
long pages. A lot of tricks had to be pulled in 'ctwill-mini.ch' to
avoid these problems, but raising others.
